WORKSHOP REGISTRATION

I, as a voluntary participant in the Every Man's Battle Intensive (the “Intensive”), agree to release and discharge, and agree to indemnify and hold harmless, New Life Ministries, its officers, directors, employees, agents, and subcontractors against all actions, causes of actions, claims, demands, costs, expenses, and liabilities of any nature whatsoever that I may suffer directly or indirectly during the course of, or as a result of, my participation in the Intensive.

I certify I am 18 years of age or older.

If I cancel my registration 14 days or more prior to the Intensive, I’ll receive a refund of the amount I’ve paid minus a $200 cancellation fee per person. I understand if I cancel less than 14 days before the start of the Intensive or I “no show” for the Intensive, no refund will be given. To cancel I must call (800)NEW-LIFE and speak to a Ministry Service Representative.

In order to maintain a sense of community and accountability, all attendees are required to stay at the hotel hosting the program and have a roommate. No daily commuting is allowed.

This Intensive is not psychiatric treatment and no doctor-patient or therapist-client relationship is established by my participation in this event.

This Intensive adheres to a Christian-based philosophy, which will influence the teaching and counsel I receive in this program.

Throughout the Intensive I will breakout into a small process group facilitated by a credentialed New Life Network Counselor. Failure to be present and on time for all small groups can be disruptive to the group process. I agree to be on time and fully participate in each group breakout meeting.

The staff of the Intensive are mandated reporters and are required to report to the proper authorities any of the following:

A. Any first or second-hand knowledge of physical or sexual abuse of a minor.

B. Any danger a participant in the Intensive may pose to a third party.

C. Any danger a participant in the Intensive may pose to himself or herself.

I will abstain from the use of alcohol and illegal drugs during the Intensive.

If I have been advised by my physician or psychiatrist to take medication, I will continue using my medications as directed during the course of the Intensive.

I will cooperate fully with the staff of the Intensive and I understand that failure to do so may result in expulsion from the Intensive.

I understand any and all information received at the Intensive is confidential.

For the privacy of all in attendance, and to safeguard the copyright of the content presented, recording or photos of any portion of the Intensive are strictly prohibited.
